What is the extraordinary function of joint cartilage?
One of the extraordinary functions of articular cartilage is its ability to replenish its major compounds in response to the mechanical forces placed on it.

Cartilage (the "cushion" at the end of the bones where they come together to form a joint) is composed of specialized cells called chondrocytes surrounded by a gelatinous matrix of collagen and proteoglycans. This matrix of immense molecules gives cartilage its property of elastic strength and resilience.

  • Collagen, the tough protein that physically connects the tissue, provides the elastic strength of cartilage.
  • Proteoglycans consist mainly of hyaluronic acid, “the glue” that holds cells and glycosaminoglycans (GAGs) together and is responsible for the normal high flexibility of synovial fluid (the fluid secreted by the lining of the joint to nourish and lubricate the joint). The water-structuring properties of proteoglycans provide the compressive resilience of cartilage.

The constant and ongoing process of cartilage manufacturing and remodeling tremendous amounts of collagen and proteoglycans often generates extremely large demands for these building blocks. If their raw materials (nutrients) are not available in the amounts required, the synthesizing process is impaired and the cartilage loses its ability to replenish itself.

How do training and exercise affect the joints, cartilage, synovial fluid, and connective tissues?
Submerged in synovial fluid, articular cartilage performs its main function by providing a smooth friction between bone edges to prevent abrasive wear and damage that may eventually result in bone fusion.

The titanic forces applied to the joint cartilage during training and competition can cause rub-off and splitting of the joint cartilage, resulting in compromised physical performance of the skeletal joints, cartilage, synovial fluid, and connective tissues in the stifle, knee, hock, and fetlock, stifle. Any of these problems can severely affect athletic equine performance regardless of how well trained, muscular, or biomechanically perfect your horse is.

If your horse develops equine osteoarthritis—sometimes caused by stresses like training and exercise—concentrations of hyalorunic acid are reduced in the cartilage and in synovial fluid, and its properties in osteoarthritic joint fluid are abnormal, comprising smaller, less shock-absorbing molecules.

What is equine osteoarthritis?
Equine osteoarthritis is the painful and destructive inflammation and enzyme production that causes a horse’s body to literally consume its own joint-added cartilage and joint lubricants. It is caused by stress, competition- and performance-related activities, trauma, or age, and is characterized by joint pain, joint stiffness, and muscle pain.

Equine osteoarthritis primarily affects weight-bearing joints in horses, including:

  • The pastern joint and coffin joint (where it is referred to as ring bone when more severe)
  • The hock (ankle—where it is commonly referred to as bone spavins)
  • The fetlock
  • The spine (neck and back—less common)
  • The stifle (less common)

Osteoarthritis does not usually affect other equine joints, except where previous injury has occurred or areas where there has been repetitive or abnormal stress.

How is equine osteoarthritis currently treated?
Treatment is essentially limited to the use of a few anti-inflammatories, mainly corticosteroids and phenylbutazone (bute). The anti-inflammatories help limit current damage but do nothing to protect or restore ravaged areas, and some of them, especially phenylbutazone, when used at the sustained dosage needed for continued athletic performance, can cause harsh side effects, including gastrointestinal erosion, ulceration, and formation of kidney lesions. Recent studies have shown that corticosteroids and phenylbutazone may also cause further damage to joint cartilage.
